Required Documentation - 2023 APPLICATION PERIOD CLOSED

Your application will not be considered completed until all of the information is submitted and received by April 7, 2023 at 4:30pm.

1. Copy of your birth certificate
   
2. Copy of government issued identification card with a photo, such as driver's license, government issued identification card or passport.
   
3. Official, sealed, high school transcripts with graduation date, or an official high school equivalency (GED) certificate and test scores. If an applicant has not yet completed high school or GED coursework at the time of application materials are due, then the applicant must submit a letter of intent to graduate, certified by a school official and on school letterhead and submit the required transcripts or GED certificate and GED scores as soon as it is available.  Your transcripts will likely be sent directly to us from your school.  They should be sent to:  JATT, 489 Stevens Street, Jacksonville, FL 32254
   
4. Applicants with military experience must submit their DD214. 
   
5. Pay a $25 application fee (NO CASH ACCEPTED). Fee must be paid by April 7, 2023 at 4:30PM.  If you paying by Check or Money Order it should be made out to JATT.  You can pay by credit card by calling the JATT at (904) 781-2112.  If you are requesting a fee waiver, please contact the office at 904-781-2112 and the required forms will be explained to you. 
   

1. You may email copies of your Birth Certificate, Identification Card, and DD214 (if applicable) to: applications.apprenticeship@gmail.com OR
2. You may mail copies of your Birth Certificate, Identification Card, and DD214 (if applicable) to: JATT, 489 Stevens Street, Jacksonville, FL 32254
3. You may drop off copies of your Birth Certificate, Identification Card, and DD214 (if applicable) at one of our two locations: 
   A) JATT, 489 Stevens Street, Jacksonville, FL 32254.  Should you wish to drop off your documents in person in Jacksonville you can do so between the hours of 9a-11:45a and 1:30p-4:45pm Monday through Friday.
   B) JATT, 2420 NE 17th Terrace, Gainesville, FL 32609.  Should you wish to drop off your documents in person in Gainesville we are only open on Wednesday's from 12p-5p.
